• The contract shall be prepared in 4 copies.

• The company title shall be written on the blank line on the first page of the contract.

• The “Healthcare Institution” section on the second page of the contract shall be filled in completely.

• Each page of the contract shall be signed by the Authorized Officer of the Healthcare Institution.
